The waist measurement is your true waist which is generally where it is the narrowest and is about two finger width underneath where your rib cage ends. Your hip measurement should be measured at the widest point. This point will vary depending on your body shape.
When measuring make sure the tape measure is perfectly horizontal around your body and firm against your skin but not squeezing in.
